OARS Instructions

To make a report on OARS follow these procedures:

1) Visit the website of Occupational Safety and Health UiTM Shah Alam

2) Click on the "Make Online Accident Report Page"

3) Fill in all the particular such as "Name of person injured", "Date of accident", "Time of accident","location of accident","Student/Staff id of person injured","Name of person making this report","Last Name of person making this report","Student/Staff id of person making this report" "Email Address of person making this report", "Telephone Number of person making this report", and "Report"

4) In the "Report" section user should enter information such as ,"Type of accident" and "Description of accident "

5) Once finished, click on the submit button.
