What to Report

 

UiTM requires the reporting of all accident, which includes; a sudden or unforeseen event which could cause or has caused, an injury or occupational illness; or a release of hazardous materials to the environment; or a using the web-based Online Accident Reporting System (OARS).Report all work-related injuries and illnesses accident to your supervisor as soon as possible. If you have an accident at work, you need to report it to your employers as soon as possible. If you can't report the accident because you are too ill, try to get someone else to do this.

 

The Following types of Accident Must Be Reported:

1) The death of any employed or self- employed person, which was caused by an accident during the course of their work

2) Any injury resulting from electrical shock or electrical burn leading to unconsciousness or requiring resuscitation or admittance to hospital for more than 24 hours

3) Loss of consciousness

4) Acute illness which requires medical treatment where there is reason to believe that this resulted from exposure to a biological agent or its toxins or infected material

5) Results from an accident arising out of or in connection with work

6) Poisoning caused by certain chemical substances

7) Explosion or fire causing suspension of normal work for over 24 hours

8) Accidental release of any substance which may damage health

9) Road Traffic accident that occurs while a person is commuting either to or from work.

 

When Should Report an Accident.

A workplace accident should be reported to the FKE OSH committee as soon as possible. The FKE OSH committe can be contacted by using one of the following methods:

Who Should Report ?

1) FKE Employees  - MUST report any accident within the vicinity of the faculty

2) Students - MUST report any accident within the vicinity of the faculty

3) Visitors or Volunteers  - MUST report any accident to OSH FKE as soon as possible.
